Transcript Request Form
To request an official college transcript from the previously attended campus, follow the steps below:
Download the transcript request form from the previously attended campus.
Complete and sign the request form.
Include a money order or check payable to Orange Technical College.
Mail your written request to the address provided on the selected form, along with a copy of your driver’s license, state ID, or other form of identification showing your name and date of birth.
NOTE: Transcript copies are $5.00 each.
GED transcripts and diplomas may only be obtained from the Florida Department of Education.
Please Note: GED transcripts cannot be requested through OTC campuses. Please do not send transcript request forms or money orders to our campuses as we are unable to fulfill these requests.
