Transcript Request Form

To request an official college transcript from the previous attended campus, follow the steps below:

Download the transcript request form from the previous attended campus.

Complete and sign the request form.

Include a money order or check pay to Orange Technical College.

Mail your written request to the address provided on the selected form, along with a copy of your driver’s license, state ID, or other form of identification showing your name and date of birth.

NOTE: Transcript copies are $5.00 each.
